The basic concept
Nobel Biocare describes the All-on-4® treatment concept as a fixed full-arch prosthesis supported by four implants: two straight anterior implants and two posterior implants tilted up to 45 degrees to make use of available bone and avoid anatomical structures.
When clinical criteria are met and the implants achieve adequate primary stability, a fixed provisional restoration may be placed on the day of surgery. That does not mean biological healing is finished that day.
Typical sequence
- Evaluation. Medical and dental history, exam, panoramic imaging and often CBCT/3D imaging.
- Treatment planning. Decide whether extractions, grafting, immediate loading or alternative approaches are appropriate.
- Implant placement. Four implants are placed for the arch in the branded treatment concept.
- Provisional prosthesis. Selected patients may receive a fixed temporary restoration immediately.
- Healing/osseointegration. Bone heals around the implants over time.
- Final prosthesis. The definitive bridge is fabricated, fitted and adjusted.
CBCT gives the surgeon a 3D view.
Cleveland Clinic notes that cone-beam CT imaging can show bone, nerves and soft-tissue structures and help guide implant placement. The exact imaging and planning workflow depends on the clinician and case.
The first set of teeth may not be the final set.
Ask whether your package includes a provisional restoration, a definitive final bridge, and which materials each uses. Timing and design vary by case and provider.
